Exposing the aged brain to anesthesia may contribute to transient and/or long-lasting neurocognitive impairment. Perioperative neurocognitive disorders are a major brain health concern for older surgical patients. The International Perioperative Neurotoxicity Working Group recommends that baseline cognitive status be assessed for older surgical patients. The American Society of Anesthesiologists’ Perioperative Brain Health Initiative advocates for the identification and evaluation of novel screening tools to predict, identify, and track brain health in older surgical patients. The ideal cognitive screening tool for baseline cognitive function would be: valid, reliable, automatically scored, and void bias. No such tool has been identified to date. This presentation reports findings from a proof-of-concept study evaluating a novel electroencephalogram (EEG) headset for assessing and tracking neurocognition in older surgical patients. These findings are important because EEG headsets might enable anesthesia providers to rapidly perform a valid and reliable neurocognitive assessment to predict the likelihood POD, perioperatively risk-stratify patients, and improve detection of mild postoperative cognitive impairments currently missed by questionnaire-based assessments (e.g., Montreal Cognitive Assessment (MoCA) & Confusion Assessment Methods (CAM)). A device capable of predicting, identifying, and tracking perioperative brain health may facilitate the development of brain protection protocols and clinical pathways to optimize neurocognitive outcomes.
