Heart failure with preserved ejection fraction (HFpEF) is an often unrecognized and underdiagnosed disease process which can seriously impact anesthetic and surgical outcomes. It is now increasingly understood that patients with diastolic dysfunction can exhibit symptoms of heart failure while presenting with normal ejection fractions (EF). In recent years, HFpEF is a disease entity that has gained attention for its' complex symptomatology and for the difficulty in treating it. There are significant anesthetic implications and risks for these patients due to their comorbidities and the unrecognized heart failure that can easily progress catastrophically if not managed correctly.
